Resmed P10/ Smartstart
When I use my P10 mask with my Resmed airsense the smart start/stop acts differently. If I have the Large pillows on it works fine. When I use the middle pillows that feature will auto start but will not stop. The directions with the mask say that that feature may not work with that mask. However this is my question. If it can't tell that the mask is taken off how can it be sensitive enough to auto titrate the right pressure throughout the night? Any thoughts

It's still sensitive enough to know what is going on when you are attached.
What happens when you remove the mask there is some residual resistance just from the smaller size nasal pillow and that little bit of resistance at your pressures with that mask is enough to make the machine think you are still attached.
So that little bit of resistance inherent in the pillow is enough to keep the machine from turning off automatically. It still thinks you are attached.
I have the same thing happen with my P10. It also happened when I was using the S9.
If you doubt the accuracy turn off SmartStart for a week and just manually turn it on and off and see if there is any huge difference in the reporting or response.
It's common and no big deal and doesn't affect what the machine does or records when you are attached.

I have used Smart start for YEARS.
With small pillows/breath volume, whatever; smart stop--doesn't.
I always manually turn my machine off.

It's perfectly 'sensitive' enough to know what's going on with your breathing, but with smaller pillows, it has trouble telling the difference between a REALLY large leak, and mask off.
wholly different from telling whether you're breathing and not breathing, and obstructed vs central apneas.
