Centrals?

Post by Rioric » Thu May 09, 2024 6:57 pm

Hello everyone! I was wondering if it looks like the CAs on my chart point to Central Apneas? Or do they look like false flags? Any insight would be greatly appreciated!

Looks like you weren't asleep to my eye.
Maybe just pausing your breathing while awake????? We do it all the time and not realize we are doing it.
If that were my report I would ignore them and chalk it up to SWJ Sleep/Wake/Junk false positives.

Thanks, I appreciate the quick response. That's what I was thinking for these too, as these were early on in the night. Do the mid-sleep ones look any different to you? It's possible I woke up for a bit there as well without realizing it....

The middle of the night snippet.....that's a weird grouping but if you look at what happened before the first flagged central you can obviously see the big gulp of air prior to the first flag and we know that we don't take big gulps of air like that when we are asleep so the first one flagged is fairly obvious SWJ. Some of the others as well though maybe not as obvious.

Anytime I see some obvious arousal/awake breathing just prior to a flagged event it makes me suspect everything after that until I see for sure asleep breathing.

See the snippet below....A big mess of centrals and can't blame it on SWJ either. Asleep for sure when all that crap happened. I am still trying to sort all that crap out. Never had that happen before.

I believe they are CA's, whether you were asleep or not. I'll bet your Flex or EPR is on. Turn it down if you can. All my opinion. I know they say 'only if asleep'... I've induced awake CA's in my self using EPR, your carotid bodies don't care if you're asleep or awake.

For it to be a central apnea that really matters you have to be asleep when it happens.

Hold your breath for 15 seconds....that's essentially a 15 second central apnea...the airway is open but no air is moving but that's because you aren't even trying to breathe. You are pausing your breathing while awake or half awake and the machine senses the "no air flow and no effort" and then it flags a central....but you weren't asleep.
Did it hurt you at all to hold your breath for 15 seconds?

But I wasn't holding my breath... believe me, I know how to breathe normally when laying in bed awake on CPAP. I induced CA's in myself using EPR. It's easy to do, I urge everyone to try it. Put on 10cm CPAP, EPR 0 for a while, then turn on EPR 3 and see if you notice. You can probably do it sitting up.

I wasn't talking to you. I was talking to the OP.

Your "experiment" is meaningless.

Pugsy, isn't that perfectly formed waxing & waning sinusoidal breathing called Periodic Breathing? When it occurs with that many CAs at the nadir and for 10 seconds or more, OSCAR reports it as CSRs (Cheyne-Stokes Respirations) on my charts, even though it is likely not true CSRs (rather, PB). I have Atrial Fibrillation (associated with CAs in the medical literature) & moderate Tricuspid Regurgitation so I do get worried when a block of CSRs are reported, but it doesn't go on all night and only happens 1-4 times a month so I don't worry much. No use reporting it to my cardiologist since he knows little or nothing about Sleep Apnea).

Anyway, strange that YOU would see this breathing pattern out of the blue.

Yeah sure looks like PB to me but the machine didn't flag it. Unsure why it didn't.
I am still trying to figure it all out. That was definitely a one off night for me. Never have had that happen at all in the past. Not in nearly 15 years being on cpap.

Right now it is on the back burner. Got some family stuff to deal with right now before I have a nervous breakdown over it all.
