please note that the post talks of sleepyhead as well as Oscar. sleepyhead was a predecessor. but the steps are still needed to get the info the forum needs.

Just asking...

Are the AHI figures posted by the O.P not considered to be a pretty good result?

I would like to know what the "patient" started at before therapy and how does the person actually feel.

Person switched from a nasal mask to a full face mask. Person is less dozy during the day. Person is still tired at times throughout the day.

I am hoping to get to an AHI under 1 if possible by adjusting the settings.

It has been so long the person would need to start all over/does not have or remember their old settings/sleep study anymore. Due to cost, it is not practical. Hence I'm trying to figure out what their machine is set at and if adjusting the settings would help.

Whoa! It took them *to the Moon* (pressure) immediately... lots of OA. The leaks look pretty good for these high pressures, that's good!

At this point it looks like the machine is fighting some severe obstructions. These pressures are considered high, close to the machine's limit of 20cm. Is this person a back sleeper? Sure looks like it.

Biggest AHI change is going to happen with side sleeping. Elevating the torso-head is next. Is the sinus clear? Do we have allergies? These can be medicated. Does the patient have an overbite? Jaw-movement devices are available. When the pressures start hitting the limit we have to do external things like this.

Just a simple test, you might try raising the minimum pressure from 4 cm. to around 12 cm.
(By steps, for patient comfort)

I would leave the max EPAP where it is.
Nice thing about APAP is the ability to titrate fairly simply, and over time.
