Thank you so kindly for enlightening me on proper American terminology in the year 2020. However, if i had meant "black American", I would have written black American. I meant neither.

"Black" Americans and Americans in general are a mixed lot. Many Americans that are considered "black" have Caucasoid features. Take for example one of my most admired Americans, Dr. Carson:

He would definitely make use of the medium cushion.

Nevertheless, this is an international forum (that means people from outside America, like the OP, are participating here). Negro or Negroid facial features would be appropriate terms referring to sub-saharan lineage in this context.

If you downsize from a medium to a size small, do you need a new headgear piece too?

No, there is only one size headgear for the N30
