I ordered an MRI with a patient who had severe OSA diagnosed in his mid 20's . His mother told me she saw him have apneas as a baby. Lo and behold his MRI showed no lacunar infarcts or any damage at all.Thats over 25 years of severe nocturnal desaturations

Yes many downs kids are prone to sleep apnea because of the macroglossia

Tongue reduction surgery should help a lot and the sooner the better.it carries a risk of post op haemorrhage so they probably will do a temporary trachy. How is his mood ?

BAD SLEEP , awaking unrefreshed , and anhedonia/depression could be a feature of depression not just sleep apnea .
