AirCurve ASV Auto/Aerophagia

Hi all,

Been awhile. So... I've been having luck with my sleep therapy, however not so much with aerophagia... please look over my data and see if there's anything you might tweak. Open
to any and all suggestions. Settings, etc. Wonder if I could get by with positive results with less pressure (if that's the problem). Thanks so much.

It’s tough when you need the higher bursts for treating the centrals but the higher burst causes the aerophagia issues. You really can’t lower your EPAP because it is already pretty much as low as it will go and the machine only goes up when it really thinks it needs to.

I would suggest talking with your doctor about maybe limiting that big burst when it is responding to the centrals. I am not sure if it would even help the aerophagia all that much and could mean sub optimal treatment of the centrals.

EPAP is the most likely offender but you settings are already pretty much at the minimum.

My machine used to blast me pretty powerfully. But over time, it seems to have learned me because it hardly ever happens now. Come to think of it, I think we reduced the backup rate. My average is 18.2 BPM but we reduced the forced backup rate to 8 BPM.

I had a new sleep study done in 2017 and it reconfirms all the 2009 findings. The one thing the Dr thought was that the backup rate was low. But I remember what a struggle it was to have it blast me just as I was falling asleep. My local DME assured me that a forced backup rate 8 BPM would not croak me and its been mostly smooth sailing since.

Just got a bit of great advice from my Sleep Tech/DME. She said that in most cases the number cause of aerophagia is dry mouth/nose. Both of which cause excess swallowing in order to alleviate the dryness. I know I do this often. It is NOT caused by leaks... in my case anyway. Changing humidifier and temp settings on the machine just might do the trick. I had mine set to Auto. Something she never recommends. SO... I'm starting at level 5 on the Humidifier and 82 degrees for the temp
and dialing them up +1 a day and see what happens. I'll post back the results. Fingers crossed.

Best of luck. My DME tech mentioned dry mouth as a cause of mouth breathing about 15 years ago.

I have to keep my acid reflux under control to limit tummy air. Diet change and nexium and incline bed.

There were times when I'd wake up and it was like my stomach was a third lung!

First you need to fix your leaks, you are wasting your time with leaks and ASV. The machine will max out trying to get treatment pressure. If you can't get the nasal to work, go full face
When you have done that, post another chart and if you want to, pm me if I don't notice
For now you can also try min epap 5.6 and max ps 11, If this chart is your average 95% pressures. That should help with the gas for now, it isn't for long term, but may get you over the hump.

Just circling back on my results... changing humidifier and temp settings on the machine has shown a MARKED improvement, if not complete resolution.
I have little to no swallowed air. First night, it was nominal, then I raised each setting by 1 for the next two nights... and voila.
It's gone. Easy fix.
