I don't recall waking up at all last night (yay!). I expected my leakage numbers to be better than this. I don't think it's mask leakage this time because that always seems to wake me up completely. In the AM, my mouth is almost always untaped on either the top or bottom. I think maybe I am drooling or something and the masking tape adhesive can't hang. I will probably try a chin strap thing from amazon. I turned EPR back on to level 2 also. This is because I was fighting against the incoming air to exhale. Hopefully that isn't messing my numbers up. I know someone had mentioned to turn it off. Either way, I have no idea why I didn't wake up with a leak of 40 or what would cause that. I have also noticed that on 2 occasions now, the auto-off feature didn't work.

Auto off sometimes doesn't work so great if the pressure is on the low side and you are using nasal pillows or similar mask.
My AirSense 10 works great with the auto on part of things but won't turn itself off if the pillow mask is attached. Will work if I remove the pillow though.

You need to learn to evaluate the leak numbers an graphs. The 40 something leak number is the maximum it went to and it wasn't up there very long. Looking at the leak graph the time in large leak was minimal and I wouldn't worry about it if it were me and I slept right through it. Quit relying so much on the numbers on the leak graph and learn to look at the graph itself and eyeball how much time you spent in large leak.

How long did you lay in bed awake this morning before you turned the machine off?

Can't really tell for sure just from this report but based on experience...that last cluster of apnea events during the last 20 minutes of so of the night... I suspected was probably not real and instead was SWJ. The bulk of your AHI is based on that cluster. If you removed that cluster your AHI is quite low.

It's why I say people can't put all their eggs in the AHI basket. Any awake/semi awake flagging has to be removed from the evaluation.
Now if you had told me "no, I woke up and immediately turned the machine off" then I wouldn't be blaming SWJ.
I suspected SWJ events just from when they happened.

The struggle continues.
If I recall correctly, I woke up from a leak around 5am and spent the next hour or so lying there trying to go back to sleep. The last hour of data is probably junk. I think I woke up numerous times before 5 also. I still sleep better with no machine. Still trying to train myself to sleep only on my back. At this point I see this as the only solution. I have no idea how people are sleeping on their sides or stomachs in ANY mask without getting leaks. I'm still not even sure if my leaks are all from the mask or from my mouth coming open because the tape can't hold it.

I went to the doctor and told him I’m struggling to stay awake by noon every day for about the last 8 years. He looked at the back of my throat and said I was about a 3 (if I recall) on the mallampati score. He then ordered a bunch of blood tests to rule out everything else.

Are you asking because my test score is so low? I would think that even waking only 1x per hour is enough to cause major changes in a persons life.

I know you don't believe me but people do sleep quite well on their sides or even on their stomachs will all of these masks.

I believe you. I’m just not sure how they achieve it. I keep going to bed saying maybe tonight will be different and it never is. The mask(s) are all fine and comfy on my back but turning my head always creates slack on one side, tension on the other, and a leak. Taping my mouth doesn’t hold through the night and everyone says the chin straps don’t really work.

I still haven’t tried a full face mask yet. I haven’t tried any of the dental appliances I’ve heard about. I haven’t tried your idea of making a custom pillow yet. What would you start with? A hunk of foam or something?

Why am I doing therapy??? By order of my MD I guess. My undersnding is that I have basically 2 choices:

Get rid of the Apnea by way of miracle, dental device, or using a CPAP
Or
Risk dying in my sleep, increased heart disease risk, diabetes risk, blood bressure, loss of brain function, irritability, inability to concentrate, inability to stay awake, falling asleep driving, etc, etc, etc,,

Go to WalMart and get a cheap memory foam bed pillow. Cut out your own little holes for the mask to rest if you are on your side.
An electric knife or even a box cutter will cut the foam easily.
Look at the various cpap pillows available to get an idea what the end result looks like.
Quit worrying about any leaking that doesn't wake you up for now.
Your biggest problem right now is not being able to sleep anywhere near soundly with the mask.
Do whatever it takes to get comfortable with the mask, any mask.
