I would guess that the doctor's policy is more about staying out of the account payment stuff. He probably doesn't want to be dragged into a billing dispute.

Use a mask liner to help with the leaks. I use RemZZZs, but many others like the Pad-a-Cheek liners.

I was using the F&P Zest, and found it was a bit noisy. I switched to the Zest Q, which is much quieter.

I like the Fisher & Paykel FlexiFit. I've also heard good things about their new Simplus mask, although I've not tried it yet. But really, it all depends on which one is the best fit for your face.
