Speaking for Kaiser through California Choice insurance (Covered CA is likely very similar): Kaiser has two different DME Formulary Lists, one is the Basic and the other is the Extended. Unfortunately all of their Individual and Small Group Plans use the Basic list, even if you have a platinum plan....
