Cpap treatment

Post by Macjacket » Fri Mar 20, 2020 4:30 pm

Guys
I’ve been on cpap for nearly 3 months now first 2 months I was on trials but now I’ve bought my own SleepStyle machine . I recently completed a titration study 2 weeks ago and was given a prescription for 15 pressure. After 2 weeks not feeling any better I took the machine back to the supplier and they downloaded the data from the USB key and said I was averaging 15.1 ahi per hour . So they increased the pressure to 17 . I’m using a resmed f20 mask and don’t have any problems sleeping. But just feel sleep deprived when I awake like I haven’t slept .
Yet I sleep an average of 8-10 hrs .
I’ve downloaded the software and am now averaging 12.1 ahi . From my understanding the target is <5 . I do have a deviated septum and can barely breath through my left nostril. Wondering if this has anything to do with it . I did try a nasal mask during my trial an eson 2 but found the air was just blowing out my mouth so tried a chin strap but my side said I was ‘blowing bubbles’ from my lips .

I’m wondering if there is something else going on ?
Is it possible I have complex sa ?

I’m at my wits end not coping with lack of sleep ( feeling rested )

Any suggestions/ recommendations greatly appreciated.

What is the AHI you are seeing composed of?
It might be complex sleep apnea but it also might simply be the obstructive sleep apnea isn't being well treated.
Are you sleeping soundly for the bulk of the night or spending a lot of time with the mask and machine on..tossing and turning and awake?

I reckon I sleep soundly occasionally I wake to go to the bathroom or move the mask

Would they not have picked up CA on the titration study ? Although I don’t have much faith in it as it prescribed 15 pressure but a week later after looking at my data they immediately increased it to 17 ( where I purchased the machine )

Without knowing what kind of events you are having it's impossible to do anything but guess.
You would think they should have picked up on complex sleep apnea in the titration study but I have seen them miss it.
Get a copy of your sleep studies (both of them) and see if they mention centrals at all.

Otherwise I am not sure if your machine even distinguishes between central apneas and obstructive apneas.

First thing you need to find out is what that AHI is made up of before you can do anything about it.
To fix a problem you first have to identify the problem.

does the machine only have usb or does it have an sd card slot as well?

not trying to be nit picky. oscar IS complex. i don't understand it myself. however, it will give you a breakdown of your events. and if you post the charts here, then the smart folk here would be able to give you advice on how to get your settings working for you.

but if it doesn't have the sd card slot, i don't see how oscar could work.

good luck!

Sorry the software I have is called sleepyhead. The machine only has USB but I’ve even tried copying it to a local folder . I’ll have another go and try post the results. I’ve asked for a copy of the titration study .

What happens if you click on "more details" or just do a daily report?

I don't know if SleepyHead/OSCAR is compatible with the SleepStyle machine.
The F & P Icon yes....but I don't know about the SleepStyle.

I Managed to upload my data usb key to the F&F site . It shows more detail than the phone app . Seems like I am still having a large number of hypopnias and some Centrals See attached pics named using the date format (US) how many centrals are acceptable ?

one thing thats always confused me is when the machine reports an AHI score does it mean treated apnoes and hypopnias or residual (untreated )?

The machine can only report the events that slipped by. Not what it prevented.

I assume your machine is only straight pressure. Is there any chance it can be set to auto range? It looks like you need more pressure. And we can't tell if it is certain parts of the night or all night long.
