2 days ago, I bought ResMed's AutoSet for Her. I was gifted an AirFit P10 starter pack, and the technician installed the Medium for me. My Dr. prescribed 4-8 cm H2O, and I felt asleep quickly.
But I felt the Medium P10 was too big for my nostrils, and switched to the Small. Yesternight, even at 8 cm H2O, I wasn't receiving enough air to inhale and my lungs started to hurt. I then increased the pressure to 6-20 cm H2O. Only then could I fall asleep. The machine displayed a pressure of 9-11 cm H2O.
So what exactly happened? What did my changing from the Medium to Small AirFit P10 cause?

The overall volume difference of the Small nasal pillow and the Medium nasal pillow is actually quite small but it is enough that some people feel like they can't move enough air with the small. I don't know why you had a problem with 8 cm and didn't with the 6 to 20 setting because you were using 6 cm while awake.
Is the ramp feature on your machine turned on by some chance? When ramp is engaged you are only getting 4 cm to start with no matter what the settings are. I suggest turning ramp off if you find it is turned on.
I don't know what all your settings are but maybe adding in EPR to the mix would help you with the not enough air flow feeling.

To know if ramp is turned on...you have to go into the clinical menu area to turn it off.

Can you describe why you felt the mediums were too large?
Remember, the pillows are not supposed to actually go into the nose. The openings should just be breaking the plane of the nostril openings. A lot of novices try to insert the pillow into the nose. This can result in leaks, blisters, discomfort, and crimping of the pillow which can also explain reduced flow.
