Switch from Airfit F30 to P10 AHI increase

Hi all

I recently switched from the F30 to the P10 nasal pillows as I find them more comfortable to wear, but my AHIs have jumped up from around 1 up to about 6 AHI.

It's been just short of a week and most of the new events are listed as Central Apneas. I was feeling pretty exhausted earlier in the week but have started to feel a little better now, although the AHI is still up.

I made some changes last night - increased min pressure from 9 to 10, turned EPR off (was on 3 full time). I'm hoping it will settle down but wanted some opinions from others. Should I just switch back?

I have attached my OSCAR data for the past 3 nights - any insights much appreciated.

I am using a Resmed S9 Autoset. Current setting min 10, max 20, ramp off, EPR off. Have been using CPAP for around 5 years now

For me the F30 always give worse AHI than the P10.

It seems to me you're mouth leaking when using the P10 - and its possilble those leaks are disrupting your sleep.

Why did you use the F30, and why did you switch to a P10?
How does your leak look on the F30?

Have you tried taping your mouth shut when using the P10?

I love pillows masks, but can't get any benefit from them if I don't mouth tape.

So basically I tried a bunch of masks when I originally got the machine, including the P10, but settled on the F30 as it gave me the best results.

My F30 headgear is a bit worn out and have always found it a bit of a nuisance to get a good seal anyway and as I had the P10 still I decided to give it another go before I replace the headgear.

I find the pillows far more comfortable and less leaky so it would be great if I could find a way to carry on using them.

Never tried tape but am willing to look into it - my leaks were actually much worse with the F30 possibly/probably due to facial hair

Thinking back I believe I tried the following

P10: Similar issues to now but only used for a couple of days. Maybe didn't give it enough of a chance
Mirage Quattro FFM: Good results initially but uncomfortable, started getting tired again after using for a few weeks
AirFit F30: Best results as far as AHI and restfulness is concerned but would constantly have to readjust and the velcro straps are annoying. Also had a fairly constant leak rate (don't have the data in front of me but was relatively high a lot of the time)

Keep forgetting to add - I found a little hole in the end of my tubing which I imagine is contributing somewhat to the overall leak rate. I will be using a fresh tube from tonight so we'll see how that goes

If you don't have to breath through your mouth, try taping.
Cover Roll tape,
3M Kind Removal Silicone tape and Medvance Silcone tape are all good, listed from the least gentle to the gentlest.
Remember to fold in a corner or a tab (or do that on both sides) to make quick removal easy.

That shared leak line looks a lot like mine when my hose had a teeny tiny hole in it.
I suspect that the baseline leak line will be much cleaner with the new hose that doesn't have a hole in it.

I have no idea what is going on with the AHI and P10. I would expect it to be the other way around and the F30 use causing a higher AHI given all other settings were unchanged.

Might be time to get a SleepHQ report shared so we can zoom in and see if those flagged events are asleep events or arousal/awake events.
Are you waking more often during the P10 nights???? If so, any idea why?

Thanks I didn't know about SleepHQ until today so will upload my data when I get home

Im not sure if I'm waking up more or less tbh my sleep is interrupted somewhat most nights anyway. But I have definitely been spending more time in bed since switching, and have noticed my dreams are more vivid and I'm feeling less rested

If you are remembering dreams then you are either waking up during the dream or right afterwards.
So remembering dreaming is actually a symptom of poor sleep quality. We don't remember (or aren't supposed to) dreams unless we wake up and it is the wake up that is the problem.

Now why we wake up....million dollar question. Even when we know the "why we wake up" it isn't always a slam dunk to fix the problem.

Yeah I figured that was probably the case about the dreams.

I'm going to give it another night or 2 with the pillows. I have some tape for my mouth too so I'll give that a go and see what happens. In any case I've ordered an Amara View headgear from amazon for the F30 as I hear it's better than the stock resmed one. So hopefully that will be the answer to my problems if the P10 turns out to be no good

OK so just to follow up on this. I tried another night with the P10 on Friday and I am 100% sure mouth leaks are the issue. Woke up multiple times due to that.

So I'm giving up on the P10. I bought some KT tape to try but honestly it just feels unsafe to tape my mouth closed.

I got the Amara View headgear I ordered the other day and it is miles better than Resmed's F30 gear so I'll be sticking with that. Used it last night with 0 leaks and apneas back down to 1.
