printable luggage tab for medical device

Several years ago, a link to a printable luggage tag for a medical device was posted. That link no longer works. I need to print a tag which says Medical Device. Can someone provide a link to such a website?

I got one custom made with my address in 2016 from these folks on ebay.. $5.. it's ok so far

That looks like a fair deal. I tried to find custom luggage tag companies to print my own design and they cost more than the one you linked to with their own design, many only offered single side printing, didn't offer custom printing of my name and address, and were on metal instead of plastic (I don't want a metal tag clinking around on my luggage).

I wanted something that wasn't ugly, so wound up just ink jet printing my own and laminating it at FedEx. It has some minor similarity to the one offered by CPAP.com because I used the same free-clip art as they did :-) But the text and layout is different and specific to my machine. This is what I printed for my ResMed A10 (with an included offset for the hole punch):

The language about the CPAP "TRTCA/DO-160G, Section 21, Category M
FAA Compliant" is true for the ResMed A10, but may not be true for other companies' devices.

So far, the tag has been completely unneeded, but I did print my name and contact info on mine just in case I manage to lose it at some point.

I also decided to make my own, based on the ones posted earlier, but with full resolution. It is a PowerPoint file, so you can either use the free PP viewer to print it, fold it, laminate it and punch the lanyard/strap hole, or use the full PP application (if you have it) to edit it to your heart's content. The attachment is a ZIP file, since pptx are not allowed, so you'll need to unzip it. I've also included a low-res JPEG so you can preview the content without having to download and open the ZIP archive.

I like both.
In addition, when traveling, be sure to put return information INSIDE each bag.
Bags get misdirected for all kinds of reasons--and outside tags can get lost.

I made a luggage tag for my daughter's instrument case on Snapfish and it is a nice heavy plastic tag. You can upload and edit a jpeg plus add printing for address and it would be sturdier than just laminating.
