Self adjusted to higher pressure and auto, higher AHI, still tired

Post by ilatruman » Thu Dec 06, 2018 11:14 am

I'm still struggling with fatigue even with using CPAP/APAP therapy for about 3 weeks now. At some forum members suggestions, I switched my pressure to 10.6 to 14 auto from 8 straight CPAP. I'm waiting for my titration study (end of this month). My AHI is showing up as higher now than it was, and from my limited experience in understanding what looks like a legitimate event and what is SWJ or a mistakenly flagged event, it looks like these events are legitimate. I'm hoping for some guidance on whether increasing my pressure was the right way to go or if I should do something different.

I will post some graphs below (my last few nights).

You have to be zoomed way in on the breathing when the flagged event happens to be able to get an idea if awake(SWJ..sleep/wake/junk) or a real asleep flagged event.

Watch all the videos
http://freecpapadvice.com/sleepyhead-free-software

Couple of quick examples.

Asleep events. Notice the nice smooth rhythmic pattern to the breaths and they don't vary much.

Normal asleep breathing....no flagged events

Asleep breathing for approx half the graph...then a bunch of irregular breathing followed by some flagged events...all those flagged events are SWJ and not real. They all happened after the arousal and the evidence of the arousal is the really irregular breathing starting right when the smooth regular pattern ends.

Here's a big overview of several minutes...pretty much every flagged event here is SWJ. This guy doesn't have OSA...he has crappy sleep with lots of spontanteous arousals documented in and in lab sleep study. He didn't sleep so great.

Yes, of course I had zoomed in. Examples below.

I'm hoping someone will be able to comment on whether this pressure looks right, and also what I can do about my leaks. Here are some more nights. I'm having a huge problems with leaks but I don't feel like I can put my mask any tighter. I turn to my side and sometimes stomach when I sleep and I have a feeling that's when a lot of the big leaks happen.

Why aren't you trying out full face masks for the leaking? Overtightening any mask is counterproductive - can even create new micro leaks and prevent silicone from inflating when it needs to.

I see my DME person tomorrow so I will ask her what she thinks. I thought full face masks were worse for leaking on your stomach or side, but I’ll look into that.

The bigger the mask, the more places there are for leaks.

If you mouth breathe, it's not a matter of mask size but of containing the leaks so that you get treated, rather than the air blowing out of your mouth. And a properly fitted FF mask does not always leak more than a badly fitted nasal one. One size (or theory) does not always fit all.

She switched me to nasal pillows, so I’m going to try that.

I tried the nasal pillows but woke up with pain on the left side of my nose cartilage/septum - has anyone ever experienced that?
