Help with mask leaks

Help with mask leaks

I keep getting low scores on the MyAir app for mask seal and events. I loaded my data into OSCAR but I don't really know what I'm looking at.

I have a F&P Vitera mask and a ResMed Airsense 10.

I don't know what I may be doing wrong. I had a virtual fitting recently and they switched me to a medium from a large. I try sleeping with a mouth guard to keep my chin from dropping. I sleep on my side and I have a EnduriMed CPAP Pillow.

it's not necessary to understand oscar. at this point, you want to be able to post graphs, so the experts here can help you.(alas, i am not among said experts.)

what you may want to do is go to youtube. search for mask fitting videos and be sure to include the name of your mask in the search field.

Thanks, I found that post after I posted but I never got notification that my post had been approved.

If I read the instructions correctly, a screenshot only shows one night at a time, right?

Here is last night's data, but it's not indicative of most nights. But it's pretty bad.

Well....parts of the night the leaks aren't so large so we know it's not a machine or hose malfunction.

The mask is moving around later in the night and it's leaking really badly when it does.

You need a major pressure adjustment in terms of the minimum and maximum both.

Once asleep you seem to go to the max of 16 and stay there. The drops during the large leak times are just the machine trying to get you to not leak so much.

Probably going to need around 14 cm minimum pressure with max of 20.

Need to fit the mask to 14 cm first at the beginning of the night and then see what happens in terms of leak later on..
It might be sweat or facial oils or just tossing and turning.

Do you have facial hair?

Are you comfortable changing the settings yourself?

If going to 14 is too big of a jump in one step make use of the ramp feature and/or go up in smaller increments.

The mask does ride up on my face often. I'll wake up and find the bottom of the mask in my mouth or on my lower lip.

I've been washing it very morning and I wash my face before bed so hopefully facial oils aren't an issue. I don't have a beard, but I don't always shave every day.

I do think I toss and turn. I have the settings all on auto and I use the ramp.

I'll reach out to my doctor about changing the pressure.

Should I post some other nights' data?

I use a similar mask and I have to tape my mouth shut to prevent this. You have the Clinician manual, you can change your pressures yourself.

Are you sure that you don't need the next size larger? Both of your pressures are too low , but I don't think that has anything to do with your mask riding up. Change minimum to 12 and max to 20. Your average is around 13.

The mask fit guide seems to indicate a medium. I don't mind posting a picture of myself with both masks if that's permitted.

Yeah, I just like to be compliant with my doctor, but it can't hurt to try it out.

What is the difference between a min of 12 vs. 14 in terms of what it does?

The minimum pressure is what does the bulk of the stenting of the the airway open in the first place. Preventing the airway from collapsing. You are also using EPR which allows the pressure to drop upon exhale and very likely you will need EPR purely from a comfort aspect since it appears you need a pressure in the mid teen range. So we need to adjust the minimum a bit to allow for that drop during exhale so that the machine can hold the airway open better.

If you sleep on your back you might try sleeping on your side to reduce your pressure needs but it isn't always so easy to stay off our backs.

Look at your pressure line...it goes up and stays up except for when the leaks were happening. I suspect if you didn't have the leaks happening that your pressure line would stay up higher even more than it does.

I was concerned about changing pressures myself when I started out but after second Dr. visit I realized they only are concerned about the compliance unless there are problems. Worst case is they tell you to go somewhere else and I don't see that happening.

Ok, I changed my settings to min 12 and max 20. Here is last night. The mask still rode up on my face several times. I'll try the large at these settings tonight.

I'm not sure how to describe this, but often I'll get a "burp" in the mask. It's sometimes my mouth pushing air out I think or other times the mask makes noises that are really annoying.
