Adjusting pressure on Trilogy 100

I'm a real novice here so please forgive my lack of knowledge. I have the Trilogy 100 as treatment for severe COPD. Mine is set at PASSIVE - AVAPS - AE, if that means anything to anyone here.

I've had a lot of trouble with it. For a long time it hasn't been providing much airway pressure at all. They finally sent someone yesterday, and I fear I may have had her set it too high. It doesn't bother me, but my breathing has been harder today after using it yesterday and last night. I almost felt like it was giving me a slight headache. When I got up this morning my eyes were all puffed up, which is not normal for me.

Can anyone offer any insight? I've been into the menu but I don't know which item I would adjust to reduce the pressure, or if the machine is set so that I even can do it.

They do take some getting use to and will self adjust each breath. They blow the pressure needed to achieve the volume of air needed each breath and what tidal volume is set.

I wouldn't change settings.
I would contact your provider to talk over how you are feeling and 9details on the settings they are using. You can tell them the details on the screen. To confirm this is what they want.

The more you know about your machine and how it works the better you are able to communicate with your provider.

There are a handful of people here familiar with the Trilogy 100. I've been using mine since 2010. It took me close to a year, and the help of people here, to get my settings optimized. I use AVAPS (not AE) so I'm pretty familiar with the settings. And there are a LOT of settings, many of which could be the culprit in why the treatment is not comfortable for you. However, I cannot make suggestions for changes to your settings. Your medical team must be contacted/involved with that.

That being said, I strongly encourage you to research and learn, learn, learn about your condition and how the Trilogy is supposed to assist. Learn about all the settings available and try to convince your medical team to let you to more involved so you can help them help you.

When I first got my Trilogy, my idiot RT (sorry RT's, but she was an idiot) had my Tidal Volume set to 600 and let the IPAP max out to get to 600. I get that probably doesn't mean a lot to you right now, but trust me, it was AWFUL. Working with the RT, we lowered the Tidal Volume by tiny increments. I was MISERABLE.

After talking with my doctor and convincing her I knew what I was talking about (thanks to people here), she let me lower it a lot (to 400) and that began my journey to optimizing my treatment. My Trilogy is still set at 400. But please remember, there are a TON of settings that can play a part.

After contacting your medical team, try to find out exactly how your Trilogy is set. I know AVAPS-AE, but we need to know the rest of the settings in order to even hypothesize about what might need adjusting.

Passive circuit just means it requires using a mask that is vented, as opposed to using an active circuit where you can attach an exhalation valve to expire the CO2 and thus can use a non-vented mask.

AVAPS-AE is a standalone therapy mode on the Trilogy, which is effectively a combination of S/T mode, AVAPS algorithm, Auto-EPAP and optional Auto-Backup Rate.

It would be very difficult to make recommendations for your scenario. But you could try typing up and listing, or take a picture, of all of the settings that are currently enabled. Ideally we can just make suggestions or questions to bring to your medical team.
