Anyone with an AHI during there sleep study of 80 or higher?

Post by My_Dream » Tue Aug 05, 2008 10:27 am

I'm just curious whether anyone stopped breathing (wihich I believe is what AHI means) more then 80 times per hour, before starting on CPAP or APAP. If so I'm just curious as to what pressure you're presently at.

Thanks.

I had "close to 700 events" in 4 hours. My longest apnea event was 55 seconds. Blood O2 levels dropped to 72%. My prescription is at 7.

My machine's LCD shows a 7 day average and 30 day average of AHI.
I started off at 28, and over the last 3 weeks its dropped to 14 (that's the 7 day average). I been on CPAP for 3 weeks now.

My Dx was AHI of 102, 99 of them apneas with longest apnea duration over 1 minute (don't remember exact number). O2 desats down to 60%.

I was titrated at 10 cm in sleep lab during split study.

Currently have APAP set to 11 min and 14 max with AFLEX at 3. If I use CPAP mode it is set to 12 cm to maintain same 0.3 AHI average.

AHI long-term average maintained at about 0.3 with zero AHI about 2 or 3 times a week.

Been on the hose two years and 4 days. I took control of my treatment within a couple of weeks of starting and never looked back for help from the so-called sleep professionals (thanks to the wise forum members here).

Kumbakaran:

Your pressure sounds way too low. Some others can just pipe in here but your AHI should be 5 or less. At 14 yours are still to high.

Hey guys...what do you think.

Depends on whether Kumbakaran has leaks under control. The treated AHI number does not mean much unless leaks are under control.

My machine's LCD shows a 7 day average and 30 day average of AHI.
I started off at 28, and over the last 3 weeks its dropped to 14 (that's the 7 day average). I been on CPAP for 3 weeks now.
I would reset your LCD data to see if you get more accurate results. If the machine had some test data on it prior to you getting it, it would skew your data. Your 7 day avg. AHI should be around 5. I would increase pressure by 1 cm to 8 and observe new AHI.

Keep your leaks under 50 L/m as shown on the display. If you reset the LCD data, it will show you the 1-day result. If you are looking for avg.s to know how you are doing the 7 day is a good one to go by.

Your Pro is just like the M series Auto except it doesn't have the Auto mode settings or Aflex.

1. Hold down both the left and right (<-) (->) buttons while simultaneously plugging the power supply into the back of the device. Once the device beeps twice, it is in the setup mode and you can release the buttons.

Navigation:
-Press the plus (+) key to enter setup OR left and right (<-) (->) buttons to enter Data Menu.
-Use the left and right (<-) (->) buttons to navigate to the next/previous screen.
-Use the (+) and (-) buttons to adjust settings or change values.
-When you have finished modifying the settings, press the On/Off button to exit setup mode and return to User mode.

Additional Settings Info/Descriptions:
Mask Alert: This alert will indicate when the device has detected a mask leak. You can enable or disable the alert (default=disabled).
Split Night Time: This setting indicates the amount of time spent in CPAP therapy before transitioning to Auto-CPAP therapy (default=Off, 120, 180, or 240 minutes).
Show AHI/Leak: You can select weather or not the Apnea/Hypopnea Index and System Leak averages display on the LCD data screens (default=disabled, enable this).
Patient Reminder: Using the EncorePro software, you can set a customized text message that will periodically display to the User. The Reminder screen lets you specifiy how often the reminder will appear (every 90 days, 180 days, etc.).

Settings:
Therapy Mode=CFLE
CPAP=8 (your pressure)
Flex Setting=2 (default=1, Off, 1, 2, or 3)
Ramp Time=30 (default=0:05-45 minutes)
Ramp Start Pressure=6.5 (default=4 to Auto:Min, only appears if Auto:Min >4 cm)
Mask Alert Feature=Off (default=off)
Auto Off Feature=Off (default=off, should be off doesn't work that well)
Show AHI/Leak Feature=ON (default=On, you want this on to get LCD data)

To view/reset data:
1. Hold down both the left and right (<-) (->) buttons while simultaneously plugging the power supply into the back of the device. Once the device beeps twice, it is in the setup mode and you can release the buttons.

2. Press the left and right (<-) (->) buttons to navigate to the Therapy Usage screen to observe data, should give you AHI information for 7 and 30 days averages, should also display number of days used >4hrs per night, 90% pressure, leak etc.

To REST LCD Data to zero, while at the Therapy Usage screen hold down the minus (-) button for 5 seconds.

someday science will catch up to what I'm saying...

Not to hijack this thread. I have yet to be "titrated", my sleep doc just guessed 7cm (I guess its an educated guess at least). I back for titration in another 3 weeks.

DreamStalker:

I think my leaks are under control. I don't have the software (yet). But again the LCD screen shows a LPM of 27 to 30. And that's with a Mirage Activa nasal. The mask manual's graph is close to 26ish at 7cm.

Snoredog:

Thanks, that's some info I had to just print out and keep with the machine!

Kumbakaran,
Since the 7cm sounds like a WAG anyway, if it were me I'd probably bump the pressure up to 8cm as Snoredog suggested & see if it helps. Unless you're worried about how the doctor would take it. If you'd rather wait 3 weeks for the titration I could understand that too.

Is my machine capable of telling me what my AHI is now, if so how do I get that information?

MyDream,

I'm pretty sure the Elite is data capable. To access it, hold down the left and right buttons until the screen reads "Efficacy Data" (about 4 seconds). You should be able to navigate using the buttons (self explanitory on the machine) to access pressure, AI, HI, AHI and leak data for the previous session, 7 day average, 30 day average, 6 month average, and 1 year average (if the machine doesn't hold enough data for the time period, it will say "no data").

To see the previous day's data, you have to check before noon (on the machine clock). Otherwise, it resets itself and will show "no data".

If you hold soen the left and right buttons and the "efficacy data" doesn't show up, you will need to get into the clinicians menu and turn on the feature that lets you see this. I can't remember if it is the smart data option or just a matter of turning on the advanced menu.
