Somewhat pressing questions on the Quattro FX fill face mask
Hi - I have some comments and somewhat pressing questions about the Quattro FX fill face mask.
I initially started my CPAP therapy in September and my first mask was a Quattro fill face mask which seem to work well being pretty comfortable, but always my mouth wih the Quattro FX fill face maskd will occasionally fall out of the bottom of the mask and get dry. Consequently I never went through the expense of replacing the mask seal and instead started looking for other mask. I've been using http://www.sleepnation.tv/trialprogram where I tried the Fisher & Paykel Forma in both medium and large sizes as well as the Respironics Fullife. I initially found Quattro FX fill face mask most comfortable of all the masks I've tried. However when I looked my ResMed S9 machine software report I had terrible leakage problems with this mask and my mouth would still pull out of the bottom of this mask. Accordingly I tried to deal with these problems.
First of all this mask was not in the trial program I mentioned above so I bought it cpap.com with return insurance should I need to try still more mask. I went to my health plans respiratory therapist and showed him my new mask he suggested I needed larger headgear. He said even though you might measure out for a medium size in this mask most men require large headgear. Reminding him about my mouth dropping out of the bottom of the mask problems he suggested I move to a large size in the mask even know according to the measuring tool supplied with this mask I am exactly the facial size for a medium mask. My chin indent occurs immediately below my bottom lip. This respiratory therapist claims this is unusual and consequently if I am having my mouth full out of the bottom of the mask problem I should try the large size.
So I noted the opinions of this respiratory therapist. When I got home and went to sleep last night I initially slept only a few hours and woke up briefly with the mouth falling out of the bottom of the mask problem I mentioned. I tried tightening the bottom straps of their headgear to the point of slight discomfort, hoping this would help the mask seal follow the mouth movement . Well that seem to work better than I would ever hope for. I slept well for the rest of the morning till almost 10 and when I created a software report got the best results I have ever gotten with that tightening adjustment. If you study the report I am posting below you will see what I mean. At a little after four in the morning I wake up briefly having experienced tremendous leakage problems while I was sleeping. After the tightening I sleep very well and get very good results according to the report I am posting below. My pressing question is if you think that I should followed that recommendation of my respiratory therapist and try a large size in this mask. If I act quickly I can have both the large and medium sizes simultaneously at home with return insurance on both. Of course this would be expensive but less expensive than having a mask that will not meet my needs. I have also asked cpap.com if I could use my insurance just to exchange the headgear size. They said since the price of the mask without headgear plus that gear alone is the same as what I have already paid for this mask I could do that with my insurance. So what do you think is a little discomfort with slightly overly tight bottom headgear straps is good as it is probably going to get or is it worth acting quickly and having both sizes at my disposal to see if I can eliminate this little discomfort?

Sounds like a good idea to try both masks at once and return the least effective.

Hi BB, I am not really good at reading graphs, but the bottom strap should not be tight enough to be uncomfortable. It kinda of defeats the whole purpose of this mask.
You can try placing the medium sized mask slightly lower on your nasal bridge so that you have a little more of the cushion below your lip. The lower part of the cushion is supposed to sit midway between your lower lip and the tip of your chin, which is where the chin indentation normally lies. Mind you, I do have to tighten the lower strap more that I do on the old style Quattro, but I can still place at least one finger under the straps. And I do need to use padacheek strap covers for the lower straps.
Also make sure that the strap that goes over the top of the head does not sit too far back.
This mask takes a lot of fiddling around with before getting it adjusted just right. Good luck.

You can solve this problem cheaply and easily with a chinstrap.

Since I last posted on this thread I tried on my mask again and do believe it is conceivable that I may need to stabilize the straps to make the mask sit right all night. The medium-size seems it may be possibly the correct size but I am not sure yet. Ms.Snuffleupagus is your purpose in using the padacheek strap covers to stabilize their position? Last night the night after the gathering of the report I posted I failed to grab another report, but the mask did ride up right to the top of my nose during the night.
I do have a chinstrap on hand which I purchased just in case that was the answer. But I thought perhaps that should be a last resort solution, but I just don't know.nobody wrote:You can solve this problem cheaply and easily with a chinstrap.

I use the padacheek strap covers to prevent marks on my face not to stabilize things. It makes the straps more comfortable.

kempo -those are great videos, every one who gets the Quattro FX fill face mask should watch them. It does make the sizing very clear to me. And I now do believe I have the right sized mask with the wrong size headgear.
I am wondering does anybody use any sort of adhesive tape product paste down this straps to one position. If one wanted to do this does anybody think there is a specific product or method which would be best?Ms.Snuffleupagus wrote:Hi BB,

I've used the quarto FX since I started on xpap. I started with the large and have changed to a medium. The problems I have are I have an oily face and when I sleep with the humidifier set to 84° the mask rides up my face with the bottom of the mask in my mouth with large leaks. I have made a mask liner from an old T shirt and for the last two nights my AHI is much lower, my leaks are slightly higher, but the mask doesn’t ride up my face and I’ve slept better.

this image from the from the ResMed FX full face mask fitting video, which can be found at the link provided to us by kempo above, is what is convincing me I should abandon my plans of obtaining both the medium and large sizes of the ResMed FX full face mask fitting just to return one of them. It seemed that this might be necessary to get the best possible fit however in the picture above the mask is being fitted on the man somewhat higher on the man's chin then when I do the same thing. This is making me think it should just be a matter of adjusting the straps and/or perhaps taping the straps and getting the proper size head gear but not obtaining a size mask that will falls on my chin below what is shown on the model above. I just thought I would check one more time with you all. Does anybody think it is worth investigating fitting a mask contrary to manufacturer recommendations. I mean putting a mask so that the mask bottom falls beneath where it does in this picture from ResMed which is the recommendation of the manufacture?

It looks right on the model in the photo. That's about where my mask is, right below my lower lip. However when you sleep your muscles relax and your jaw could drop down causing leaks. A larger mask may not even work out for you. I tried a larger one and it ended up being so big it sat on my eyes.
How did the chin strap work, did you try it? I would strongly suggest giving that a try before a whole new mask size. I really think the chin strap will solve the problem for you.
Also don't see what taping the headgear straps is going to do. They should stay in place with the velcro.
