Not related to cpap but could be coincidental. Hiatal hernias hurt lying down and then slide back down when you are upright. they feel like a fist in your back orlike a muscle spasm???

I think Elle is right.. after 6 months now of trying to figure out my back pain and trips to chiroprctors etc. Three nights ago I tried sleeping with Wedge Pillow to keep my upper half elevated and guess what.. no more lung/spasm pain at all. I am going to keep using the wedge and see if things stay the same. I think the thing that drove me the most crazy was that the pain would go from upper to middle and then back up and down from night to night but I couldn't pin it down. It is strange how things develop once you start to really sleep again.

Have you tried a wedge pillow? I was amazed that it worked after just the first night.
Actually helps you breathe a bit better too.
