First Two Nights on CPAP (ResMed AirSense 11) – Any Tips or Things I Should Watch For?

Hi everyone,

I’m new to CPAP therapy and just completed my second night using a ResMed AirSense 11 Autoset with the AirFit™ P30i Nasal Pillow Mask. I’m feeling pretty comfortable so far, but I’m curious if there’s anything I should be aware of or any improvements I can make to my setup. Here’s some background and my current experience:

Why I started CPAP therapy:
I originally visited my ENT because I was experiencing sinus headaches, particularly on days when it rained. He recommended an at-home sleep study to rule out any underlying issues. The sleep study revealed a Respiratory Event Index (REI) of 34.1, which breaks down to:
  • 7 obstructive events per hour
  • 27 hypopneas per hour
  • 0 central or mixed apneas
  • 0 RERAs
This was a bit of a surprise to me since I didn’t have the typical symptoms like snoring, daytime sleepiness, or trouble focusing. My spouse never noticed any signs of stopped breathing either. The only other notable health issue I have is hypertension, which is controlled with medication and improving with weight loss and exercise. I also manage generalized anxiety, but it’s well-treated.

My CPAP Experience So Far:
I’ve used the machine for two nights now, sleeping for around 7.5 hours each night—the same as I usually did before starting CPAP.
  • I generally feel comfortable using the CPAP. The only mild issue is some slight irritation around my nostrils, which might be due to my moustache rubbing against the nasal pillows (I have a beard).
  • I did wake up at least once each night while on CPAP, though I don’t remember why. This is similar to what would occasionally happen during more anxious periods in the past (1-2 times a week).
  • I haven’t noticed any major changes in how I feel during the day, but I might have a small increase in energy. I’m not sure if it’s real or just placebo, as I didn’t feel tired before starting CPAP.
CPAP Data So Far:
  • Night 1: AHI of 2.43
  • Night 2: AHI of 2.92
This seems decent, but I’m not sure if there’s room for improvement or if I should aim for a lower AHI.

  • I’m curious if there are any adjustments I should make to my setup or routine to improve my experience, particularly around reducing the nasal irritation.
  • Should I be concerned about the AHI numbers I’m seeing, or is this normal for starting CPAP?
  • Overall, I’m happy so far and not planning to change anything unless there’s a good reason to, but I’d love to hear what others think!

You are getting some big pressure swings, especially on the first chart. If those charts were mine, I would change the minimum pressure to 7.0 and check another night. 6.0 for the first experience if you are the cautious type.

Watching some YouTube videos on fitting and adjusting your pillows model may help eliminate this. If the irritation continues, applying a small amount of Lanisoh lanolin can help. Don't worry about your pronouns. https://www.walmart.com/ip/Lansinoh-HPA ... s/17206542

That's great! Several awakenings per night is normal. They only become a problem if it takes a long time to fall back to sleep.
